Community First Health Plans is partnering with Councilwoman Shirley Gonzales and the American Diabetes Association to help local residents start 2015 with an affordable health plan through the Health Insurance Marketplace.
On Monday, Dec. 15, these partners will join SRA International, Inc., University Health System CareLink and Enroll America to host a seven-hour marketplace enrollment and health fair. The event will include free glucose screenings, educational seminars, and assistance in enrolling in affordable health coverage to treat and prevent chronic illnesses, such as diabetes, asthma and heart disease.
Anyone who enrolls in a health plan on Dec. 15 will be covered starting in January 2015. You still have until Feb. 15 to get coverage during the open enrollment period.
HOW TO ENROLL: Bring the following: Social Security numbers, dates of birth, resident card/work visa/citizenship certificate/number, tax return/W2/check stub/unemployment letter/social security award letter/bank statement, current health insurance card (if any), username and password for Healthcare.gov account (if you have one).
